Frank Smith Award 2009
The Sheffield Occupational Health and Safety Association is proud to present the Frank Smith Award.
This award acknowledges the most engaging and proactive initiative with potential wider application within the Association.
If you would like your organisation to be nominated for this award, we are looking for examples of creativity and innovation involving your employees in the following areas:
- Schemes for promoting health and safety awareness;
- Tackling specific areas of risk;
- Promotion of a healthier working environment.
Eligibility for entry
You may nominate your own organisation or any other organisation whose initiative is considered worthy of this award. Making applications and assessment criteria.
It will be necessary to complete all four sections of the entry form within the word limitations indicated, be sure that the name of your organisation and required information is provided and that all supporting documents are identified and referenced.
The attached marking sheet is for your guidance only and is designed to enable you to understand the marking criteria. Note that the greatest marks can be gained for your methods of organising and implementing the initiative, the actual benefits achieved as well as potential for wider application within the Association.
Confidentiality
All applications will be assessed by the committee and thereafter your details will be kept in accordance with the relevant Data Protection Acts. Please restrict including individual names in your application to assist with matters of confidentiality.
Closing Date & Submission of Application
Please ensure that we receive your entry no later than 1st January 2009, by posting or delivering to:

Announcement of winning entry and presentation of award
The winning entry will be contacted by telephone not later than 1st March 2009 and the trophy will be presented during the Association AGM on Tuesday, 10th March 2009. The committee will publish details of the result on the Association website and in the RoSPA journal.
